How to Request a Change to a Life Insurance or Annuity Policy

If you are contemplating any change to your life insurance or annuity policy, we strongly urge that you first contact your State Farm agent who will be happy to assist and advise you.

If you prefer, you may initiate a change by completing, printing, signing (if required), and mailing the appropriate Request Letter below.
